PROTECTION OF PERSONAL INFORMATION ACT PARENTAL CONSENT FORM

By consenting this , and unless you at any time instruct the School expressly and in writing to the contrary, your consent is given for the School to :

• We hereby agree that it is with our full understanding and consent that as a School with Christian Ethos, Bible teaching and prayer forms part of the school curriculum.

• All pupils are subject to the system of the discipline and the rules in force at Precious Lighthouse Academy.

• The School post photographs with all children`s picture on their social media pages and press releases to celebrate the school`s or your child`s activities, achievements and successes.

We hereby indemnify, hold harmless and absolve the Principal and the Staff of Precious Lighthouse Academy, Elsies River, acting in good faith as agent of the Governing Body , all claims whatsoever as may arise from accident of injury to, or any loss or damages of the above-named pupil.

I / We the Undersigned, agree that in the case requiring emergency medical attention that the school may take your child to the nearest Doctor, Hospital or Day Hospital.

1. Registration fee and school fees are paid on admission.
2. School fees are paid in the month advance
3. All school fees must be paid on time [ Before the last day of each month]
4. Full Year School fees are paid on a monthly basis and full amount are due end of the year.
5. As an Independent / Private school we operate at own cost, meaning we are solely dependent
on parents paying school fees. Consequently, if parents neglect to pay fees for their children’s
Education, the school in turn, cannot pay the educators responsible for that Education.
6. Non-payment of fees is a breach of contractual obligation by the signatory and is enforceable
by law to pay the full year school fees.
7. Full month school fees are required whether child has attended school or not.
8. We understand and accept that any fees in arrears will bear interest of 5% per month.
9. One month written notice is required in the event of discharge. If not, parent is liable for the full
remainder of contract.
10. The contract between parent and school is binding.
11. The school will hand outstanding accounts over for collection and this may result in listing on
the credit bureau.
12. Admin / Re-Registration fee of R350.00 are paid yearly [ Payable end of October ].
13. December & January fees are due end of November each year.
14. No cash fees, all fees must be paid into school bank account or via card machine at our
Reception Desk.
